We are seeking highly qualified, part- and full- time Licensed Practical Nurses (LPNs) to join our team to deliver excellent care for our residents. The ideal candidates will have senior living experience (e.g., long-term care, assisted living, memory support) with a passion for working with an elderly population.

7:00 AM - 3:00 PM
3:00 PM - 11:00 PM
11:00 PM - 7:00 AM
KEY RESPONSIBILITIES:
Provide direct nursing care per care plans across CCRC levels of care (independent living, assisted living, skilled nursing), supporting physical and psychosocial well-being.
Perform assessments, document findings, and report condition changes to RN/physician and care team.
Monitor vital signs, weights, I&O, and blood glucose; administer medications, injections, wound care, and ordered treatments within scope.
Implement restorative care, including positioning, ADL training, fall prevention, and range of motion exercises.
Assist residents with ADLs (bathing, dressing, toileting, grooming, transfers, and ambulation) using safe techniques.
Support engagement in recreational, social, and therapeutic activities to promote quality of life.
Coordinate safe admissions, transfers, and discharges within the CCRC, ensuring continuity of care.
Collaborate with interdisciplinary team members to develop and update individualized care plans.
Educate residents and families on medications, disease management, and health maintenance.
Follow infection control standards, safety regulations, and facility policies.
Maintain accurate and timely EHR documentation of care, assessments, and outcomes.
Support and guide CNAs and support staff as needed to ensure quality resident care.
Requirements
REQUIREMENTS:
High school diploma or GED required.
Current Virginia LPN license required.
Current Adult CPR and BLS/AED certification required.
Minimum of three (3) years of progressive experience in CCRC, long-term care, home health, case management, or related healthcare setting.
Strong commitment to service excellence, resident safety, and high-quality care.
Excellent communication, organization, time management, critical thinking, and multitasking skills.
Self-motivated with ability to follow verbal and written instructions accurately and efficiently.
Compassionate, patient, and professional in providing care to geriatric residents.
Respects resident rights and maintains confidentiality and discretion with protected health information.
Knowledgeable in nursing standards, clinical procedures, safety protocols, laws, and regulatory guidelines.
Able to work independently and exercise sound clinical judgment in routine and urgent situations.
Proficient in EMR systems, Microsoft Office, and standard clinical/office technology.
Able to meet schedules, deadlines, and reporting requirements consistently.
